Dermoscopic Features of Bowen’s Disease and Their Diagnostic Value: A Clinical Observational Study

Abstract

Objective: To systematically analyze the dermoscopic features of Bowen’s disease (BD) and explore their differences across various anatomical sites and their diagnostic value. Methods: We prospectively collected data from patients with histopathologically confirmed BD between January 2024 and August 2025. Dermoscopic images were recorded, and the prevalence of features such as background erythema, keratinization characteristics, vascular patterns, and pigmentation patterns were analyzed. The dermoscopic presentations of BD on the head/face were compared with those on the trunk/limbs. Results: A total of 65 BD lesions were included. The most common dermoscopic features were back-ground erythema (91.0%), yellow-white/keratotic scales (89.0%), irregular linear/serpentine vessels (74.0%), and glomerular vessels (74.0%). Follicular-related features (e.g., perifollicular white halo, follicular keratotic plugs) were observed in head/facial BD, whereas glomerular vessels, dots/globular vessels, and clustered brown structureless areas were significantly more common in trunk/limbs BD (P < 0.001). Conclusion: BD presents with relatively characteristic dermoscopic patterns that vary depending on the lesion site. Dermoscopy serves as a valuable non-invasive tool in the diagnosis and differential diagnosis of BD.
